Why this combination exists
Sildenafil and tadalafil dominate the combination market. Vardenafil pairings are rarer, and they are aimed at men who have already worked out that vardenafil is the molecule that suits them — and who also finish too quickly.
Switching to a sildenafil combination just to get the delay agent would mean giving up the drug that works. This avoids that trade.
The vardenafil half
It inhibits PDE5, keeping the vessels in erectile tissue open while arousal continues. Onset is typically twenty-five minutes to an hour, with a window of four to five hours.
It also handles an ordinary meal better than sildenafil does, which is one of the reasons men prefer it.
About the dapoxetine half
This is the part that handles early ejaculation. Dapoxetine keeps serotonin available in the nerve synapse for a short period, and since serotonin is what restrains the ejaculatory reflex, the reflex arrives later.
It is a short-acting drug by design — in and out within hours, taken only when wanted. Realistic gain is a multiple of your current time, which usually means minutes rather than half an hour.
Two things to take seriously. Dizziness and fainting on standing happen, so drink a full glass of water with the tablet, get up slowly, and skip alcohol and driving. And do not combine it with MAO inhibitors, thioridazine, linezolid, lithium, St John's wort, other SSRI or SNRI antidepressants, tramadol or triptans.
The heart rhythm caution, which matters more here
Vardenafil slighly prolongs cardiac conduction. Dapoxetine can cause dizziness and fainting through a drop in blood pressure. Neither is dramatic alone; together they deserve more respect than a single-ingredient tablet.
If you take amiodarone, sotalol or quinidine, or have known QT prolongation, this tablet is not for you. Full stop.
When should you take it?
One tablet one to three hours before sex, with a large glass of water. The lead time is set by the dapoxetine half.
One in twenty-four hours and not every day. Stand up slowly, do not drive, and skip alcohol that evening.
Which side effects are common?
From vardenafil: headache, flushing, blocked nose, occasional dizziness or nausea, brief visual flicker.
From dapoxetine: nausea, dizziness, dry mouth, insomnia, sometimes diarrhoea. The blood pressure dip on standing is the one that actually hurts people.
One combination you cannot have
Nitrates. That covers nitroglycerin in any form, isosorbide mononitrate, molsidomine, riociguat, and poppers. Both substances widen vessels by the same route, and together the blood pressure fall can be severe enough to stop the circulation working.
No gap between doses makes it acceptable. Anyone prescribed nitrates for angina needs a different approach to erection problems, and a doctor is the right person to find it.
Contraindications
Heart disease, heart failure, conduction disorders, recent heart attack or stroke. A history of fainting, epilepsy, mania or severe depression. Severe liver impairment, dialysis, inherited retinal disease.
Do not combine with MAO inhibitors, thioridazine, linezolid, lithium, St John's wort or other SSRI or SNRI antidepressants. Ritonavir and some antifungals raise vardenafil levels sharply.
If only one problem applies
Then take one drug. A combination tablet means two risk profiles for a benefit you only need half of, and there is no advantage in that.
