How does it work?

Two ways to take it

Side effects, including the odd one

Nitrates: the hard no

Nitroglycerin spray or tablets, isosorbide mononitrate, molsidomine, riociguat for pulmonary hypertension, and poppers from the recreational shelf. All of them open blood vessels through the same pathway this drug uses. Put them together and blood pressure can drop far enough to collapse the circulation.

There is no spacing trick that makes this safe. If you carry a nitrate spray for chest pain, this class of medicine is not for you, and that is worth saying plainly rather than burying in small print.

Who needs to avoid it?

Recent heart attack or stroke, unstable angina, severe heart failure, uncontrolled blood pressure high or low. Severe liver or kidney impairment needs an adjusted dose. After sudden loss of vision in one eye, the drug is ruled out.

Alpha blockers for prostate symptoms need care, since both act in the same direction. Ritonavir, ketoconazole and clarithromycin raise levels; rifampicin lowers them.

What erection problems can be telling you

The vessels involved are narrow, so they show damage early. High blood pressure, diabetes and raised cholesterol sometimes announce themselves here first, years before anything else.

If the problem is new and persistent, have blood pressure and blood sugar checked. A tablet covers the signal; it does not fix what is behind it.
