What is in the sachet

A flavoured gel containing sildenafil citrate, usually one hundred milligrams per portion, made by Ajanta Pharma. Vol-1 denotes a particular set of flavours; the drug content is the same across the assortments.

Each sachet is sealed separately, which makes carrying one unobtrusive.

Why a gel at all

Two reasons. There is nothing to swallow, which matters to a surprising number of people. And part of the dose is taken up through the lining of the mouth, so the effect often begins sooner — frequenly within fifteen to thirty minutes.

The trade-off is precision. A sachet holds a fixed amount and does not reseal once opened.

How to use it

Tear the sachet, squeeze the gel into your mouth, let it sit briefly, swallow. No water needed. Half an hour before is usually about right.

One sachet in twenty-four hours. If the full strength is more than you want, a lower-strength tablet is a better route than trying to save half a sachet.

How does it work, exactly?

Sildenafil blocks the enzyme PDE5. During arousal the vessels in erectile tissue widen; the enzyme would reverse that quickly, and the drug prevents it.

Arousal is the starting condition. The gel supports what is already happening; it does not initiate anything.

Food, drink, timing

Fat slows absorption here too, though less than with a coated tablet. A heavy meal right before is still a poor idea.

Alcohol in quantity lowers blood pressure twice over and flattens arousal. A glass of wine bothers nobody; an evening in a bar does.

What side effects should you expect?

Headache, a hot red face, blocked nose, heartburn. Quicker absorption sometimes makes them more noticable at first, and they tend to clear faster as well.

Occasionally light seems harsh or colours look faintly blue. Sudden vision loss, hearing loss on one side, or a painful erection that will not resolve mean the emergency department — not waiting it out.

What must never be taken alongside

Nitrate medicines — nitroglycerin, isosorbide mononitrate, molsidomine — plus riociguat and poppers. The two act on the same mechanism, and the combined drop in blood pressure can be fatal rather than merely unpleasant.

This is not a precaution that scales with dose. It applies to the smallest tablet as much as the largest, and it has no safe interval.

Other cautions

Alpha blockers for prostate symptoms need spacing. Ritonavir, ketoconazole and erythromycin raise sildenafil levels considerably.

Avoid with severe heart disease, soon after a heart attack or stroke, with very low blood pressure, with severe liver or kidney impairment, or with inherited retinal disease. Not for women, not under eighteen.

Storage

Cool and dry, though not refrigerated. Keep sachets sealed until use or the gel dries out. Do not leave the box in a car in summer — the contents turn runny.
