What is in it and who makes it

Sildenafil citrate, one hundred milligrams, from Alkem Laboratories — a large Indian manufacturer with a long presence in this market.

The brand name belongs to the company. The drug inside is the same compound that has been treating erectile dysfunction since the late nineties.

A hundred milligrams is the top, not the middle

Worth repeating because it is the most common mistake with generic sildenafil. One hundred milligrams is the licensed maximum for a day, not a standard starting dose.

A lot of men do well on half. Starting there means a milder headache while you learn how you respond, and you can always take the full tablet next time.

What does it do inside you?

Arousal releases nitric oxide. A messenger rises, smooth muscle in the erectile tissue relaxes, blood enters. The enzyme PDE5 clears the messenger away and the erection ends.

Sildenafil blocks PDE5, so the erection holds instead. Nothing starts without arousal.

Timing

Thirty to sixty minutes before, with water. Quicker on an empty stomach, slower after a fatty meal — sometimes by a full hour, which is the usual explanation for a first attempt that seemed to fail.

The window afterwards is four to five hours, counted from swallowing.

What interferes

Fat slows it. Grapefruit juice speeds it up by slowing the liver enzyme that clears the drug, which raises concentrations — leave it out on the day.

One alcoholic drink is immaterial. Several lower blood pressure further and dull arousal, so the tablet works against them.

What are the side effects?

Headache, a hot flushed face, blocked nose, heartburn. Sometimes a faint blue tinge to colours or extra sensitivity to glare. Usually worst on the first occasion and quieter after.

Sudden vision loss, hearing loss in one ear, chest pain, or an erection lasting hours and hurting all need help the same hour. The last one risks permanent damage to the tissue.

One combination you cannot have

Nitrates. That covers nitroglycerin in any form, isosorbide mononitrate, molsidomine, riociguat, and poppers. Both substances widen vessels by the same route, and together the blood pressure fall can be severe enough to stop the circulation working.

No gap between doses makes it acceptable. Anyone prescribed nitrates for angina needs a different approach to erection problems, and a doctor is the right person to find it.

Check what arrives

With low-cost generics, look at the print quality, the batch number and the expiry date. Tablets within one pack that differ noticeably in colour or shape justify suspicion.

Who should not take it
