Super Kamagra: the best-known of the dual tablets

Two problems that arrive together

More often than people expect. Anxiety about losing an erection creates urgency; urgency ends things early; the early finish costs the erection. Each half of the problem keeps the other going.

Super Kamagra interrupts both. The usual split is sildenafil for the erection and dapoxetine for the delay.

The sildenafil half

It inhibits PDE5 and keeps the vessels in erectile tissue open as long as arousal continues. Onset is around half an hour to an hour, with a four to five hour window.

No arousal means no effect, and combining two drugs does not change that.

What dapoxetine brings, and what it costs you

Dapoxetine is a serotonin reuptake inhibitor built for occasional use — it peaks inside an hour and is largely gone within a few. Serotonin acts as the brake on the ejaculatory reflex, and holding it in place for a while pushes the reflex back.

The price of admission is a specific side effect: a drop in blood pressure on standing, sometimes severe enough to faint. Take it with a large glass of water, stand up slowly, and do not drive. Alcohol makes this considerably worse, so leave it out that evening.

It also rules out a long list of other drugs. Monoamine oxidase inhibitors, thioridazine, linezolid, lithium, St John's wort and other SSRI or SNRI antidepressants do not mix with it, and a gap of weeks is needed either side of an MAO inhibitor. Tramadol and migraine triptans are risky too.

How much extra time, honestly

Trials of dapoxetine show a lengthening to a multiple of the starting time. For most men that means seconds become minutes. Anyone expecting half an hour is looking for something that does not exist.

For a man who has barely been getting started, minutes is a substantial change. And the reduction in pressure often matters as much as the pharmacology.

When should you take it?

One tablet one to three hours beforehand, with a large glass of water. Not more than one in twenty-four hours, and not every day.

Stand up slowly afterwards, and do not drive. Those are not throwaway lines — the blood pressure dip is real.

What can you expect to feel?

From sildenafil: headache, flushing, blocked nose, heartburn, sometimes visual changes.

From dapoxetine: nausea, dizziness, dry mouth, insomnia, occasionally diarrhoea or irritability.

What must never be taken alongside

Nitrate medicines — nitroglycerin, isosorbide mononitrate, molsidomine — plus riociguat and poppers. The two act on the same mechanism, and the combined drop in blood pressure can be fatal rather than merely unpleasant.

This is not a precaution that scales with dose. It applies to the smallest tablet as much as the largest, and it has no safe interval.

Alcohol is a bigger problem here

More so than with a plain erectile dysfunction tablet. Alcohol and dapoxetine amplify each other's effect on blood pressure and balance, and the chance of fainting rises measurably.

Leaving it out that evening is the sensible call, not excessive caution.

When the tablet is not an option

Heart disease, heart failure, conduction disorders, recent heart attack or stroke, a history of fainting, epilepsy, mania or severe depression, severe liver impairment.

MAO inhibitors, thioridazine, linezolid, lithium, St John's wort and other SSRI or SNRI antidepressants are all incompatible, with weeks of separation needed around an MAO inhibitor.

The part no tablet covers

Early ejaculation usualy has a psychological side. Medication buys time and lowers the stakes; it does not replace a conversation with a partner or with someone qualified.

Many men use the breathing space to practise techniques that still work once the tablet has worn off. That is the most useful way to approach it.
