Bottle or blister: what is different

The medicine is unchanged. Branded vardenafil tablets, same strength, same manufacturer. The packaging is the only variable.

A bottle is faster to open and takes up less room. A blister protects each tablet from air and moisture individually, which a bottle only does while the cap is screwed down properly. That is the whole trade-off.

How to store them properly

Cap tight, desiccant left inside if one was supplied, somewhere dry at ordinary room temperature. Avoid the bathroom entirely — steam is the main enemy here.

If tablets look chalky, discoloured or crumbly, throw them out. Loose tablets also make it easy to lose track of the expiry date, so keep the label intact.

What vardenafil does

It inhibits the enzyme PDE5. Normally that enzyme ends an erection by clearing away the messenger that keeps vessels in erectile tissue open. Inhibit it and the erection stays firm while arousal lasts.

Without arousal there is no effect whatsoever. That holds for every drug in the class.

Onset and duration

Usually twenty-five minutes to an hour, with a window of roughly four to five hours afterwards. Many men find it starts a bit sooner than sildenafil does.

A very fatty meal pushes the onset back; a normal meal does not matter much.

What dose is right?

Ten milligrams an hour ahead is standard, twenty if needed and tolerated, five for men who react strongly or are older. Liver problems mean starting at the bottom.

One tablet per day. With loose tablets it is easier to double up by accident, so count before you take.

The caution specific to this molecule

Vardenafil slighly prolongs electrical conduction in the heart. If you take amiodarone, sotalol or quinidine, or have known QT prolongation, choose tadalafil or sildenafil instead.

For men without those conditions it is not an issue. But it is the reason this drug is not a blanket first choice.

What can you expect to feel?

Headache, flushing, nasal congestion, occasionally dizziness or nausea. Brief visual flicker or light sensitivity happens.

Urgent: sudden vision loss, hearing loss on one side, chest pain, or an erection that persists and becomes painful.

Nitrates: the hard no

Nitroglycerin spray or tablets, isosorbide mononitrate, molsidomine, riociguat for pulmonary hypertension, and poppers from the recreational shelf. All of them open blood vessels through the same pathway this drug uses. Put them together and blood pressure can drop far enough to collapse the circulation.

There is no spacing trick that makes this safe. If you carry a nitrate spray for chest pain, this class of medicine is not for you, and that is worth saying plainly rather than burying in small print.

Not suitable if

Recent heart attack or stroke, severe heart failure, very low blood pressure, severe liver disease, dialysis, or an inherited retinal condition. Alpha blockers need spacing; ritonavir and certain antifungals require a reduced dose.
