What you are buying

Sildenafil citrate, one hundred milligrams per tablet, under the Fildena brand name. The manufacturer is Fortune Healthcare. The active ingredient is the same one found in the original blue tablet.

It travels on cost rather than on anything clinical. There is nothing wrong with that as long as you know what the name does and does not signify.

One hundred milligrams is a lot to start with

It is the top of the standard range, not the middle. A great many men do perfectly well on half, and starting there means fewer headaches while you find out how you respond.

What does it do in the body?

Arousal releases nitric oxide, a messenger rises, smooth muscle in the erectile tissue relaxes and blood flows in. PDE5 clears that messenger and the erection subsides.

Sildenafil inhibits PDE5, so the messenger lingers and the erection holds. Arousal first, always.

Timing and food

Thirty to sixty minutes ahead with water. Empty stomach is quicker; a fatty meal can add an hour or more. The window is four to five hours from swallowing.

Grapefruit juice raises levels and should be skipped. One alcoholic drink is fine; several work against the tablet by lowering blood pressure and blunting arousal.

What can you expect to feel?

Headache, a flushed face, blocked nose, heartburn. Occasionally a blue cast to colours or glare sensitivity. Usually worst on the first occasion.

Sudden loss of vision, hearing loss in one ear, chest pain during sex, or a painful erection that does not subside all need help the same hour.

One combination you cannot have

Nitrates. That covers nitroglycerin in any form, isosorbide mononitrate, molsidomine, riociguat, and poppers. Both substances widen vessels by the same route, and together the blood pressure fall can be severe enough to stop the circulation working.

No gap between doses makes it acceptable. Anyone prescribed nitrates for angina needs a different approach to erection problems, and a doctor is the right person to find it.

Worth a look at the pack

Clear printing, a batch number, an expiry date. Tablets within one pack that vary noticeably in shade or shape are a bad sign.

Who should not take it
