What it is

What is the mechanism?

Food is not a factor

A substantial meal barely shifts the onset. That makes the whole thing less conspicuous, and it removes one of the common reasons sildenafil appears not to work.

Several drinks still work against it, since alcohol lowers blood pressure in the same direction and flattens arousal.

The prostate benefit

Tadalafil relaxes smooth muscle in the prostate and bladder neck as well. Men with benign enlargement on a daily dose often find urination easier and night-time trips less frequent.

Several countries license the drug for that indication outright.

Back ache, and the other effects

Dull muscle pain in the lower back or thighs is the characteristic tadalafil effect — often a day late, usually gone within two. A simple painkiller is enough; if it happens every time, change molecule.

Also headache, warm face, blocked nose, reflux. All temporary. Sudden visual change, hearing loss on one side, chest pain during sex, or a painful erection that will not resolve all need help within the hour.

One combination you cannot have

Nitrates. That covers nitroglycerin in any form, isosorbide mononitrate, molsidomine, riociguat, and poppers. Both substances widen vessels by the same route, and together the blood pressure fall can be severe enough to stop the circulation working.

No gap between doses makes it acceptable. Anyone prescribed nitrates for angina needs a different approach to erection problems, and a doctor is the right person to find it.

What interacts, and when is it excluded?

Alpha blockers for prostate symptoms need care and a low start, because both work in the same direction. Ritonavir, ketoconazole and clarithromycin raise levels; rifampicin lowers them.

Excluded with recent heart attack or stroke, unstable angina, severe heart failure, uncontrolled blood pressure, or after sudden sight loss in one eye. Severe liver or kidney impairment needs adjustment.
