A different principle entirely

Nitrates are even more awkward here

Because the drug is in your blood continuously, there is no window in which a nitrate becomes acceptable. Nitroglycerin, isosorbide mononitrate, molsidomine, riociguat and poppers are all out, permanently, for as long as you take this.

That is a point worth raising with any doctor who might prescribe you a nitrate spray for chest pain.

Other interactions and cautions

Alpha blockers for prostate symptoms need coordinating, precisely because both act in the same direction. Ritonavir, ketoconazole and clarithromycin raise tadalafil levels and call for a reduction.

Not suitable with severe heart disease, recent heart attack or stroke, unstable angina, severe heart failure or uncontrolled blood pressure. Severe kidney or liver impairment usually rules out daily dosing.

Who gets the most out of it

Men in settled relationships having sex regularly, and men with prostate symptoms on top. If sex is occasional, the on-demand dose is cheaper and means less medicine overall.
