Kamagra is a trade name belonging to Ajanta Pharma. What is in the tablet is sildenafil citrate, the compound that has been treating erectile dysfunction since the late nineties.

It became widespread on price rather than on any clinical difference. Worth knowing, because the name itself tells you nothing about strength or quality.

One hundred milligrams is the top of the range

Not a middle-of-the-road dose. Plenty of men are perfectly served by half a tablet, and starting there is the sensible approach for a first attempt, for older men, or for anyone who tends to react strongly to medicines.

A tablet a day is the limit. Two at once gives a worse headache and the same erection.

What happens in the body

Arousal releases nitric oxide, which raises a messenger that relaxes smooth muscle in the erectile tissue so blood can enter. The enzyme PDE5 breaks that messenger down, ending the erection.

Sildenafil blocks PDE5. The erection therefore holds. Remove the arousal and the tablet does nothing at all.

Timing

Thirty to sixty minutes before sex, with water. Quicker on an empty stomach, slower after a heavy meal — sometimes by a full hour.

After that you have roughly four to five hours. The clock starts when you swallow.

Food, drink and grapefruit

Fat is the brake. Grapefruit juice is the accelerator, because it slows the liver enzyme that clears sildenafil. Leave it out on the day you plan to take a tablet.

A single drink is unimportant. More lowers blood pressure further and dulls arousal, which means the tablet is pulling against the alcohol.

What are the side effects?

Headache, hot flushed face, blocked nose, heartburn, sometimes a blue tinge to colours or extra sensitivity to glare. Strongest the first time, usualy milder after.

Sudden vision loss, hearing loss in one ear, chest pain, or an erection that stays for hours and hurts — all of those need help the same hour, not the next morning.

What must never be taken alongside

Nitrate medicines — nitroglycerin, isosorbide mononitrate, molsidomine — plus riociguat and poppers. The two act on the same mechanism, and the combined drop in blood pressure can be fatal rather than merely unpleasant.

This is not a precaution that scales with dose. It applies to the smallest tablet as much as the largest, and it has no safe interval.

Legible print, a batch number, an expiry date. Tablets inside a single pack that differ in colour or shape are a reason for doubt.

Who needs to avoid it?

Severe heart disease, recent heart attack or stroke, very low blood pressure, severe liver or kidney impairment, inherited retinal disease. Alpha blockers need spacing; ritonavir and certain antifungals require a reduced dose.
