Why avanafil rather than sildenafil

Avanafil was developed later and is more selective about which enzymes it touches. In practice that means a faster onset — often around fifteen minutes — and fewer of the nuisance effects, particulary the visual ones.

For a combination tablet that matters. If you are already taking on the side effects of a second drug, starting from a cleaner first one is an advantage.

What the avanafil half does

It inhibits PDE5, so the vessels in erectile tissue stay open while arousal lasts. The window runs around six hours — longer than sildenafil, far shorter than tadalafil.

Arousal is still the starting condition, as it is with every drug of this type.

About the dapoxetine half

This is the part that handles early ejaculation. Dapoxetine keeps serotonin available in the nerve synapse for a short period, and since serotonin is what restrains the ejaculatory reflex, the reflex arrives later.

It is a short-acting drug by design — in and out within hours, taken only when wanted. Realistic gain is a multiple of your current time, which usually means minutes rather than half an hour.

Two things to take seriously. Dizziness and fainting on standing happen, so drink a full glass of water with the tablet, get up slowly, and skip alcohol and driving. And do not combine it with MAO inhibitors, thioridazine, linezolid, lithium, St John's wort, other SSRI or SNRI antidepressants, tramadol or triptans.

Taking it

One tablet one to three hours before sex with a full glass of water. The timing is set by the dapoxetine component, which needs more lead time than avanafil does.

One tablet in twenty-four hours and not every day. Dapoxetine is designed for occasional use rather than as a daily medicine.

What side effects should you expect?

From avanafil: headache, flushing, nasal congestion, sometimes back pain. Less visual disturbance than its older relatives.

From dapoxetine: nausea, dizziness, dry mouth, insomnia. The drop in blood pressure on standing is the one to take seriously, and alcohol makes it considerably worse.

One combination you cannot have

Nitrates. That covers nitroglycerin in any form, isosorbide mononitrate, molsidomine, riociguat, and poppers. Both substances widen vessels by the same route, and together the blood pressure fall can be severe enough to stop the circulation working.

No gap between doses makes it acceptable. Anyone prescribed nitrates for angina needs a different approach to erection problems, and a doctor is the right person to find it.

Who needs to avoid it?

Men with heart disease, heart failure, conduction disorders, or a recent heart attack or stroke. Anyone with a history of fainting, epilepsy, mania or severe depression. Severe liver impairment.

Ritonavir, ketoconazole, itraconazole and clarithromycin raise avanafil levels substantialy. MAO inhibitors, thioridazine, linezolid, lithium, St John's wort and other SSRI or SNRI antidepressants are incompatible with the dapoxetine half.

Realistic expectations

A firmer erection that holds, and a delay measured in minutes rather than a transformation of your sense of time. Set against that: two drugs, two sets of side effects.

If only one of the two problems applies to you, a single-ingredient tablet is the better choice.
