Start with the number

The licensed maximum daily dose of sildenafil is one hundred milligrams. This product is above that. Knowing it before you swallow a whole tablet is the single most useful thing on this page.

Past the standard dose the effect does not climb meaningfully. What climbs reliably is headache, flushing, palpitations and dizziness.

Splitting is the sensible move

Half a tablet lands inside the normal dosing range and suits most men perfectly well. If you have never taken a PDE5 inhibitor, do not open with the full amount.

How does sildenafil work?

None of that is cause for panic, but it is cause for restraint. The equation of more drug equals better sex simply does not hold.

Warning signs

An erection lasting hours and becoming painful is an emergency — the tissue can be permanently injured. Sudden vision loss, one-sided hearing loss and chest pain also need immediate attention.

What must never be taken alongside

Nitrate medicines — nitroglycerin, isosorbide mononitrate, molsidomine — plus riociguat and poppers. The two act on the same mechanism, and the combined drop in blood pressure can be fatal rather than merely unpleasant.

This is not a precaution that scales with dose. It applies to the smallest tablet as much as the largest, and it has no safe interval.

Interactions are more consequential here

Alpha blockers for prostate symptoms need spacing and small amounts. Ritonavir, ketoconazole or erythromycin raise sildenafil levels further, which at this strength is a poor idea.

Who should avoid it?

Men with heart disease, after heart attack or stroke, with low blood pressure, or with liver or kidney impairment. Inherited retinal disease rules it out.

If you take several daily medicines, speak to a pharmacist before the first tablet. At this dose that is not an excess of caution.

If the standard dose was not enough

The answer usualy is not more milligrams. More often the timing, the meal or the situation was wrong. Sometimes a different molecule fits better — tadalafil and vardenafil behave differently.

Persistent difficulty deserves investigation. Erection problems are frequently an early signal of vascular or metabolic disease, and a bigger dose only drowns out the signal.
