What is in the tablet

Tadalafil and dapoxetine, from Centurion Laboratories. One addresses the erection, the other the ejaculatory reflex. They are not related drugs and they do not work in the same way.

The two halves run on different clocks

This is the detail most people miss. Dapoxetine peaks within an hour and is largely cleared within several. Tadalafil is still active well into the following day.

So the delaying effect ends long before the erectile effect does. If you want the delay again the next evening, you cannot simply take another tablet — one in twenty-four hours remains the limit, and the tadalafil is still circulating.

How the tadalafil component works

It inhibits PDE5, which keeps the smooth muscle in erectile tissue relaxed for as long as arousal is present. Food barely affects it, so timing is flexible.

There is a secondary effect on the prostate and bladder neck, which men with benign enlargement sometimes notice as a stronger stream.

About the dapoxetine half

This is the part that handles early ejaculation. Dapoxetine keeps serotonin available in the nerve synapse for a short period, and since serotonin is what restrains the ejaculatory reflex, the reflex arrives later.

It is a short-acting drug by design — in and out within hours, taken only when wanted. Realistic gain is a multiple of your current time, which usually means minutes rather than half an hour.

Two things to take seriously. Dizziness and fainting on standing happen, so drink a full glass of water with the tablet, get up slowly, and skip alcohol and driving. And do not combine it with MAO inhibitors, thioridazine, linezolid, lithium, St John's wort, other SSRI or SNRI antidepressants, tramadol or triptans.

Taking it

One tablet one to three hours before sex with a full glass of water. That lead time belongs to the dapoxetine half.

One tablet per twenty-four hours, not on consecutive days. Stand slowly, do not drive, and leave the alcohol alone that evening.

Which side effects are common?

From tadalafil: headache, warm face, blocked nose, reflux, and the characteristic dull ache in the lower back or thighs, often arriving a day late.

From dapoxetine: nausea, dizziness, dry mouth, insomnia, occasionally diarrhoea. Fainting on standing is the serious one.

One combination you cannot have

Nitrates. That covers nitroglycerin in any form, isosorbide mononitrate, molsidomine, riociguat, and poppers. Both substances widen vessels by the same route, and together the blood pressure fall can be severe enough to stop the circulation working.

No gap between doses makes it acceptable. Anyone prescribed nitrates for angina needs a different approach to erection problems, and a doctor is the right person to find it.

Who is it unsuitable for?

You have heart disease, heart failure, a conduction disorder, a recent heart attack or stroke, or unstable angina. Also excluded: a history of fainting episodes, epilepsy, mania, severe depression, or severe liver impairment.

Incompatible drugs include MAO inhibitors, thioridazine, linezolid, lithium, St John's wort and other SSRI or SNRI antidepressants. Ritonavir, ketoconazole and clarithromycin raise tadalafil levels and require a lower dose.

A note on expectations

Expect a firmer erection accross a long window, and a delay measured in minutes. That is what the evidence supports for these two ingredients.

If early ejaculation is your only complaint, dapoxetine on its own means one drug instead of two. Less medicine for the same result is usually the better deal.
