What must never be taken alongside

Nitrate medicines — nitroglycerin, isosorbide mononitrate, molsidomine — plus riociguat and poppers. The two act on the same mechanism, and the combined drop in blood pressure can be fatal rather than merely unpleasant.

This is not a precaution that scales with dose. It applies to the smallest tablet as much as the largest, and it has no safe interval.

Interactions and limits

Alpha blockers need spacing. Ritonavir, ketoconazole and erythromycin raise levels and call for a lower dose. Grapefruit juice off the list.

Avoid entirely with severe heart disease, recent heart attack or stroke, very low blood pressure, severe liver or kidney impairment, or inherited retinal disease.

Storage

Keep them in the blister and dry. Chewables draw in moisture far more readily than coated tablets, so the bathroom is the worst storage spot in the house. Heat turns them soft.
