Why splitting still makes sense

How sildenafil behaves

What is more likely at 120 mg

Headache that throbs rather than nags. A face that goes properly red. Blocked nose, palpitations, dizziness when getting up. Light sensitivity and a blue tint to colours turn up more often as well.

All of it fades with the drug. But it is the predictable cost of the extra twenty milligrams, and most men do not need them.

Signs that need attention now

An erection lasting hours and hurting is an emergency. Sudden loss of vision, hearing loss on one side, and chest pain during sex also mean immediate medical help.

One combination you cannot have

Nitrates. That covers nitroglycerin in any form, isosorbide mononitrate, molsidomine, riociguat, and poppers. Both substances widen vessels by the same route, and together the blood pressure fall can be severe enough to stop the circulation working.

No gap between doses makes it acceptable. Anyone prescribed nitrates for angina needs a different approach to erection problems, and a doctor is the right person to find it.

Which combinations matter?

Alpha blockers for prostate symptoms need spacing and a low dose, because both lower blood pressure. Ritonavir, ketoconazole and erythromycin push sildenafil levels higher still; rifampicin blunts the effect.

Who should not use it

Men with heart disease, after a recent heart attack or stroke, with low blood pressure, severe liver or kidney impairment, or an inherited retinal condition. Not for women, not under eighteen.

How should you store it?

Room temperature, dry, in the blister. Not the bathroom, not the glovebox.
