Sublingual is a specific instruction
Under the tongue, left to dissolve. Not chewed, not swallowed with a glass of water. The point is that part of the sildenafil enters the bloodstream directly through the mucous membrane and skips the stomach.
Two things follow: the onset arrives sooner, and what you ate matters rather less. Those are the only reasons this format exists.
Get the technique right
Keep it under the tongue untill it has gone. Avoid drinking for a few minutes afterwards. Swallow it straight down and it behaves like any other tablet, in which case you have paid for nothing.
The taste is average at best. It only needs tolerating for a short while.
How it works
Sildenafil inhibits PDE5, the enzyme that would otherwise clear away the messenger that keeps vessels in erectile tissue relaxed during arousal. Blocked enzyme, sustained erection.

Side effects arrive quicker
Headache, flushing, blocked nose, heartburn, sometimes visual changes. Faster absorption can make the first wave sharper, and it usually passes sooner as well.
Sudden vision loss, one-sided hearing loss, chest pain, or an erection that persists painfully all require help within the hour.
What must never be taken alongside
Nitrate medicines — nitroglycerin, isosorbide mononitrate, molsidomine — plus riociguat and poppers. The two act on the same mechanism, and the combined drop in blood pressure can be fatal rather than merely unpleasant.
This is not a precaution that scales with dose. It applies to the smallest tablet as much as the largest, and it has no safe interval.
What should it not be taken with?
Alpha blockers need spacing and a reduced dose. Ritonavir, ketoconazole and erythromycin raise levels; rifampicin lowers them. Grapefruit juice off the list for the day.
Not suitable if
Severe heart disease, recent heart attack or stroke, very low blood pressure, severe liver or kidney impairment, or inherited retinal disease.
Storage
Sublingual tablets are more vulnerable to humidity than coated ones. Keep them sealed and dry, out of the bathroom and out of a hot car.
