Worry about losing an erection produces haste. Haste finishes things early. Finishing early then takes the erection with it, and the worry grows. Men in this position rarely break out of it by willpower.

What sildenafil does here

It blocks PDE5, keeping the vessels in erectile tissue open while arousal continues. Onset lands around half an hour to an hour, with a window of four to five hours.

Arousal remains the trigger, exactly as with any sildenafil product.

The second ingredient, and its own rules

Dapoxetine was designed to be taken when needed rather than daily. It reaches its peak within about an hour and clears quickly, which is exactly why it suits this purpose. By slowing the reuptake of serotonin it delays the reflex that ends things early.

Expect minutes rather than a different sense of time — trials show a lengthening to a multiple of the starting point, so seconds become minutes. For a man who barely gets started, that is a substantial change.

The warnings matter here. Nausea and dizziness are common, and fainting on standing is the one to respect: plenty of water, slow movements, no driving, no alcohol. MAO inhibitors, thioridazine, linezolid, lithium, St John's wort and other antidepressants of the SSRI or SNRI type are incompatible, with weeks of separation required around an MAO inhibitor.

How do you take it?

One tablet one to three hours before sex, with a full glass of water. The water is part of the instruction rather than a courtesy — dapoxetine causes dizziness more readily when you are short of fluid.

One tablet in twenty-four hours, and not daily. The dapoxetine component is intended for occasional use.

What comes from each half?

Sildenafil: headache, flushing, blocked nose, heartburn, occasionaly visual changes.

Dapoxetine: nausea, dizziness, dry mouth, insomnia, sometimes diarrhoea. Light-headedness on standing is the one that causes real problems, and occasionally it goes as far as fainting.

Nitrates: the hard no

Nitroglycerin spray or tablets, isosorbide mononitrate, molsidomine, riociguat for pulmonary hypertension, and poppers from the recreational shelf. All of them open blood vessels through the same pathway this drug uses. Put them together and blood pressure can drop far enough to collapse the circulation.

There is no spacing trick that makes this safe. If you carry a nitrate spray for chest pain, this class of medicine is not for you, and that is worth saying plainly rather than burying in small print.

When this tablet is ruled out

Heart disease, heart failure, conduction disorders, recent heart attack or stroke. A history of fainting, epilepsy, mania or severe depression. Severe liver impairment.

Two drugs mean two risk profiles. That is why the list is longer than for a single-ingredient tablet, and why it deserves reading rather than skimming.

What medication cannot handle

Early ejaculation usualy has a psychological element — anticipation, pressure, a bad experience that set a pattern. A tablet takes pressure off and buys time; it does not resolve any of that.

Plenty of men use the time it buys to practise techniques that keep working afterwards. That is the most sensible way to use something like this.
