Why there is a third molecule

Sildenafil worked, but not for everyone and not comfortably for everyone. Headaches that would not shift, or an effect too weak to be useful. Vardenafil was developed as an alternative — chemically a relative, not a copy.

So an unsatisfying experience with one does not predict the next. That is the practical reason this drug matters.

Timing, which is where it scores

Onset usualy falls between twenty-five minutes and an hour, and many men find it faster than sildenafil. The window afterwards runs about four to five hours.

A very fatty meal pushes it back. An ordinary one barely matters, which is a meaningful difference from sildenafil in day-to-day life.

How it works

Vardenafil inhibits PDE5, the enzyme that ends an erection by breaking down the messenger keeping the vessels in erectile tissue relaxed. Inhibit the enzyme and the erection lasts while arousal lasts.

Take away arousal and nothing happens. That holds for every drug in this family.

Doses are lower than sildenafil's

Ten milligrams about an hour beforehand is standard. Twenty if ten falls short and was well tolerated. Five for men who react strongly, and as the starting point past sixty-five or with liver problems.

One tablet in twenty-four hours is the ceiling.

The heart rhythm point

Vardenafil slightly prolongs the QT interval, a measure of electrical conduction in the heart. For the great majority of men this has no practical consequence at all.

It does matter if you take amiodarone, sotalol or quinidine, or have a diagnosed QT prolongation. In that case pick tadalafil or sildenafil. This is a real reason to choose a different drug, not a disclaimer.

Which effects turn up most often?

Headache leads, then facial flushing, a blocked nose, sometimes dizziness or a queasy stomach. Vision may flicker briefly or feel over-sensitive to bright light.

Sudden loss of vision or hearing on one side means stop and seek help. A painful erection that will not subside is an emergency.

One combination you cannot have

Nitrates. That covers nitroglycerin in any form, isosorbide mononitrate, molsidomine, riociguat, and poppers. Both substances widen vessels by the same route, and together the blood pressure fall can be severe enough to stop the circulation working.

No gap between doses makes it acceptable. Anyone prescribed nitrates for angina needs a different approach to erection problems, and a doctor is the right person to find it.

Which other drugs matter here?

Alpha blockers for prostate symptoms are workable with spacing and a small dose. Ritonavir and several antifungals raise vardenafil levels sharply, so the dose must come down. Leave grapefruit juice alone.

Who should avoid it

Men with a recent heart attack or stroke, severe heart failure, very low blood pressure, severe liver impairment, or on dialysis. Inherited retinal disease such as retinitis pigmentosa rules it out.

If a doctor has advised against sexual exertion because of your heart, that advice is unaffected by the availability of a tablet.
