Who makes it and what is in it

Tadalafil, twenty milligrams, from Cipla — a large and long-established Indian manufacturer. Tadacip has been on the market for many years, which is part of why it is widely trusted among the generics.

The molecule is the same one in the branded tablet. A licensed generic has to demonstrate equivalent absorption before it can be sold.

Duration is what you are buying

Sildenafil gives four or five hours. Tadalafil stays active well past twenty-four. That single difference changes how the drug fits into life.

What is the mechanism?

Back ache and the other side effects

Dull muscle pain in the lower back or thighs is more common with tadalafil than with its relatives, frequently a day late and gone within two. A simple painkiller is enough.

Also: headache, warm face, blocked nose, reflux. All temporary. Sudden visual change, hearing loss on one side, chest pain during sex, or an erection that will not subside and hurts — those need help within the hour.

What must never be taken alongside

Nitrate medicines — nitroglycerin, isosorbide mononitrate, molsidomine — plus riociguat and poppers. The two act on the same mechanism, and the combined drop in blood pressure can be fatal rather than merely unpleasant.

This is not a precaution that scales with dose. It applies to the smallest tablet as much as the largest, and it has no safe interval.

What interacts, and when is it excluded?

Alpha blockers for prostate symptoms need care and a low start, since both act in the same direction. Ritonavir, ketoconazole and clarithromycin raise levels; rifampicin lowers them.

Excluded with recent heart attack or stroke, unstable angina, severe heart failure, uncontrolled blood pressure, and after sudden sight loss in one eye. Severe liver or kidney impairment needs adjustment.
