A less common format for a less common molecule

Sildenafil jellies are everywhere. Vardenafil in gel form is rarer, which makes this useful for men who have settled on this particular molecule but dislike swallowing tablets.

Why the gel starts sooner

Part of the dose is absorbed through the lining of the mouth, bypassing the stomach. There is also no tablet to disintegrate first, which removes a step.

Many men report an effect within fifteen to twenty-five minutes. The window afterwards is the usual four to five hours for vardenafil.

How vardenafil works

It inhibits PDE5. During arousal that enzyme would clear away the messenger that keeps vessels in erectile tissue relaxed, ending the erection. Inhibited, it cannot.

The caution attached to this molecule

Vardenafil lengthens cardiac conduction slighly. If you take amiodarone, sotalol or quinidine, or have known QT prolongation, choose a different active ingredient.

Food and alcohol

Vardenafil handles ordinary meals reasonably well. A very fatty one still slows things.

Several drinks lower blood pressure and dull arousal; one does neither to any useful degree.

What side effects should you expect?

Headache, flushing, blocked nose, sometimes dizziness or nausea. Visual flicker or light sensitivity can occur briefly.

Sudden vision loss, hearing loss in one ear, chest pain, or an erection that persists painfully all need help the same hour.

Nitrates: the hard no

Nitroglycerin spray or tablets, isosorbide mononitrate, molsidomine, riociguat for pulmonary hypertension, and poppers from the recreational shelf. All of them open blood vessels through the same pathway this drug uses. Put them together and blood pressure can drop far enough to collapse the circulation.

There is no spacing trick that makes this safe. If you carry a nitrate spray for chest pain, this class of medicine is not for you, and that is worth saying plainly rather than burying in small print.

Which other drugs matter here?

Alpha blockers need spacing and a low starting dose. Ritonavir and certain antifungals raise levels substantially. Grapefruit juice is best avoided.

Not suitable if

Recent heart attack or stroke, severe heart failure, very low blood pressure, severe liver impairment, dialysis, or inherited retinal disease.

Storage

Cool and dry, not refrigerated. Keep sachets sealed until the moment of use, and do not leave them in a hot car.
