Why a fourth drug was developed

Sildenafil, tadalafil and vardenafil all work, but each has an annoyance attached. Waiting an hour. Flushing. Nasal congestion. A blue tint to the world. Avanafil was built to be more selective about which enzymes it touches.

The result is a drug that comes on faster and tends to cause fewer of the visual and sinus complaints. It does not work better in the sense of producing a stronger erection — it works more cleanly.

How quickly it starts

Often around fifteen minutes, sometimes a little longer. That is noticeably faster than anything else in the class, and for men who dislike planning it matters more than the pharmacology suggests.

The window runs about six hours. Shorter than tadalafil, longer than sildenafil.

The mechanism, briefly

Avanafil inhibits PDE5, the enzyme that shuts down an erection by clearing away the messenger that keeps vessels open. Block the enzyme and the erection lasts.

Arousal remains the trigger. Nothing happens without it, which is a safety feature rather than a limitation.

Where should the dose start?

Fifty milligrams is a common starting point, with one hundred milligrams as the usual step up and two hundred available if the lower doses do not deliver. Older men and anyone with liver trouble should start low.

One tablet in twenty-four hours. Taking a second because the first seemed weak adds side effects and nothing else.

Food is less of an obstacle

A heavy, fatty meal slows absorption a little, but much less than it does with sildenafil. For most men a normal dinner makes no practical difference.

Grapefruit juice is still worth skipping on the day — it slows the liver enzyme that clears the drug and pushes levels up.

What men actually report

Headache sits at the top of the list, then flushing and a stuffy nose, sometimes back pain. The visual disturbances that sildenafil is known for are less common here.

Sudden loss of vision, hearing loss on one side, chest pain during sex, or an erection that will not subside and becomes painful — any of those means medical help now, not tomorrow.

One combination you cannot have

Nitrates. That covers nitroglycerin in any form, isosorbide mononitrate, molsidomine, riociguat, and poppers. Both substances widen vessels by the same route, and together the blood pressure fall can be severe enough to stop the circulation working.

No gap between doses makes it acceptable. Anyone prescribed nitrates for angina needs a different approach to erection problems, and a doctor is the right person to find it.

Other interactions worth checking

Alpha blockers for prostate symptoms need spacing and a low starting dose. Ritonavir, ketoconazole, itraconazole and clarithromycin raise avanafil levels substantially, so the dose has to come down. Rifampicin does the opposite and weakens it.

If you take anything daily, run the list past a pharmacist first. It takes a few minutes.

Who is it not for?

Men with recent heart attack or stroke, severe heart failure, unstable angina, very low or badly controlled blood pressure, severe liver or kidney impairment, or an inherited retinal disease. Not for women, not for anyone under eighteen.

If the first attempt disappoints

Try again before drawing conclusions. First attempts fail often, usualy because of nerves, alcohol or bad timing rather than the drug. Several goes is the sensible test.

And if erection problems are new and persistent, get blood pressure and blood sugar checked. The small vessels involved here often show vascular trouble before anything else does.
