Why the two problems travel together

Fear of losing an erection creates urgency. Urgency finishes things early. Finishing early then costs the erection. It becomes a loop, and it is hard to break from inside.

Cenforce D attacks both ends. Sildenafil handles the erection, dapoxetine delays the reflex.

What sildenafil contributes

It blocks PDE5, keeping the vessels in erectile tissue open for as long as arousal lasts. Onset falls around half an hour to an hour, with a window of four to five hours.

As always, no arousal means no effect. The combination tablet does not change that.

What dapoxetine brings, and what it costs you

Dapoxetine is a serotonin reuptake inhibitor built for occasional use — it peaks inside an hour and is largely gone within a few. Serotonin acts as the brake on the ejaculatory reflex, and holding it in place for a while pushes the reflex back.

The price of admission is a specific side effect: a drop in blood pressure on standing, sometimes severe enough to faint. Take it with a large glass of water, stand up slowly, and do not drive. Alcohol makes this considerably worse, so leave it out that evening.

It also rules out a long list of other drugs. Monoamine oxidase inhibitors, thioridazine, linezolid, lithium, St John's wort and other SSRI or SNRI antidepressants do not mix with it, and a gap of weeks is needed either side of an MAO inhibitor. Tramadol and migraine triptans are risky too.

How do you take it?

One tablet one to three hours before sex, with a full glass of water. The water is not a throwaway instruction — dapoxetine causes dizziness more often when you are dry.

One tablet in twenty-four hours, and not every day. The dapoxetine component is meant for occasional use, not as a standing medication.

What side effects come from each half?

From sildenafil: headache, flushing, nasal congestion, heartburn, sometimes visual changes.

From dapoxetine: nausea, dizziness, dry mouth, insomnia, occasionaly diarrhoea or irritability. The blood pressure drop on standing is the one that causes real trouble.

What must never be taken alongside

Nitrate medicines — nitroglycerin, isosorbide mononitrate, molsidomine — plus riociguat and poppers. The two act on the same mechanism, and the combined drop in blood pressure can be fatal rather than merely unpleasant.

This is not a precaution that scales with dose. It applies to the smallest tablet as much as the largest, and it has no safe interval.

When this tablet is ruled out

Heart disease, heart failure, conduction disorders, recent heart attack or stroke. A history of fainting episodes, epilepsy, mania, or severe depression. Severe liver impairment.

That is a long list and none of it is boilerplate. Two active ingredients means two risk profiles stacked on top of each other.

The part a tablet cannot do

Early ejaculation usualy has a psychological component. Medication buys time and takes pressure off, but it does not replace a conversation — with a partner, or with someone qualified.

Plenty of men use the time it buys to practise techniques that keep working after the tablet has worn off. That is the sensible way to use something like this.
