Why a second molecule is worth trying
Once sildenafil was in wide use, its gaps became clear. Some men got headaches they could not live with, others got very little effect at all. Vardenafil was developed as an alternative, chemically related but not the same.

The mechanism
Vardenafil inhibits PDE5. That enzyme ends an erection by breaking down the messenger that keeps the vessels relaxed. Block it, and arousal produces a firmer erection that lasts.
Doses are smaller
Ten milligrams about an hour ahead is standard, twenty if ten is insufficient and well tolerated, five for men who react strongly. Over sixty-five or with liver problems, start at the bottom.
One tablet in twenty-four hours.
The caution specific to vardenafil
It lengthens the QT interval slighly — electrical conduction in the heart. For most men that is of no consequence whatsoever.
It matters if you take amiodarone, sotalol or quinidine for an arrhythmia, or have a known QT prolongation. In that case tadalafil or sildenafil is the better choice. This is a real selection criterion, not fine print.
What can you expect to feel?
Headache first, then flushing, a stuffy nose, sometimes dizziness or an unsettled stomach. Brief flickering vision or glare sensitivity can occur.
Sudden vision loss, hearing loss on one side, chest pain, or a persistent painful erection all need urgent help.
Nitrates: the hard no
Nitroglycerin spray or tablets, isosorbide mononitrate, molsidomine, riociguat for pulmonary hypertension, and poppers from the recreational shelf. All of them open blood vessels through the same pathway this drug uses. Put them together and blood pressure can drop far enough to collapse the circulation.
There is no spacing trick that makes this safe. If you carry a nitrate spray for chest pain, this class of medicine is not for you, and that is worth saying plainly rather than burying in small print.
Which other drugs matter here?
Alpha blockers for prostate symptoms are allowed with spacing and a low dose. Ritonavir and some antifungals raise vardenafil levels sharply. Grapefruit juice should be skipped.
Who should avoid it
Recent heart attack or stroke, severe heart failure, very low blood pressure, severe liver impairment, dialysis, inherited retinal disease.
If a doctor has advised against sexual exertion on cardiac grounds, that advice does not change because a tablet is available.
