Why tadalafil rather than sildenafil in a combination

Because the window is longer. Sildenafil gives you four or five hours; tadalafil runs well past a day. Combined with a delay agent, that means one tablet can cover a whole evening and the following morning without any recalculation.

The Super designation refers to the higher tadalafil strength in this version compared with the standard one.

An important mismatch to understand

The two halves do not last the same length of time. Dapoxetine is gone within hours; tadalafil is still working the next day.

So on day two you effectively have a tadalafil-only tablet in your system. That is fine, but it means you must not take a second combination tablet to get the delay back — one in twenty-four hours is the hard limit, and taking another PDE5 inhibitor on top would be worse.

The tadalafil half

It inhibits PDE5, keeping smooth muscle in erectile tissue relaxed during arousal. Onset is half an hour to an hour; food barely affects it.

It also relaxes smooth muscle in the prostate and bladder neck, which some men notice as easier urination.

The second ingredient, and its own rules

Dapoxetine was designed to be taken when needed rather than daily. It reaches its peak within about an hour and clears quickly, which is exactly why it suits this purpose. By slowing the reuptake of serotonin it delays the reflex that ends things early.

Expect minutes rather than a different sense of time — trials show a lengthening to a multiple of the starting point, so seconds become minutes. For a man who barely gets started, that is a substantial change.

The warnings matter here. Nausea and dizziness are common, and fainting on standing is the one to respect: plenty of water, slow movements, no driving, no alcohol. MAO inhibitors, thioridazine, linezolid, lithium, St John's wort and other antidepressants of the SSRI or SNRI type are incompatible, with weeks of separation required around an MAO inhibitor.

How do you take it?

One tablet one to three hours before sex with a full glass of water. The lead time is dictated by dapoxetine rather than tadalafil.

One tablet in twenty-four hours, and not on consecutive days.

Side effects from both halves

Tadalafil: headache, flushing, blocked nose, reflux, and dull muscle ache in the lower back or thighs that may show up the next day.

Dapoxetine: nausea, dizziness, dry mouth, insomnia. Light-headedness on standing is the one that lands men on the floor, so get up slowly and skip alcohol.

Nitrates: the hard no

Nitroglycerin spray or tablets, isosorbide mononitrate, molsidomine, riociguat for pulmonary hypertension, and poppers from the recreational shelf. All of them open blood vessels through the same pathway this drug uses. Put them together and blood pressure can drop far enough to collapse the circulation.

There is no spacing trick that makes this safe. If you carry a nitrate spray for chest pain, this class of medicine is not for you, and that is worth saying plainly rather than burying in small print.

Who cannot take it?

Heart disease, heart failure, conduction disorders, recent heart attack or stroke, unstable angina. A history of fainting, epilepsy, mania or severe depression. Severe liver impairment.

MAO inhibitors, thioridazine, linezolid, lithium, St John's wort and other SSRI or SNRI antidepressants do not mix with dapoxetine. Ritonavir, ketoconazole and clarithromycin raise tadalafil levels.

Who this suits and who it does not

It suits men with both problems who want the long window. It does not suit men with only one of the two — a single-ingredient tablet means fewer side effects for the same benefit.

And it is not a good first experiment for anyone who has never taken either drug. Start with one and learn how you respond.
