An accident in cardiology
The molecule was being developed for angina. Vessels were supposed to widen and the heart muscle get more blood. During the trials a side effect turned up that sent the whole programme in another direction.
What came out of it was the first drug that reliably treated erectile dysfunction. Pfizer make the branded version; patent protection expired years ago, which is why generics exist.
Most disappointment comes from the wrong expectation
The tablet does not produce an erection. It stops an enzyme from shutting one down early. If there is no arousal, nothing happens — which also means nobody walks around for hours with an unwanted erection.
Control stays with the body. That is a feature, though it rarely gets described as one.
The mechanism in order
Arousal releases nitric oxide. A messenger called cGMP rises, smooth muscle in the erectile tissue relaxes, blood enters. The enzyme PDE5 clears that messenger away, which is how an erection normaly ends.
Sildenafil blocks PDE5. The messenger lasts longer, vessels stay open, the erection is firmer and holds. That is all it does.
Half an hour to an hour of lead time
That is how long absorption takes. On an empty stomach it is quicker; after a fatty meal it can stretch considerably. This is the single most underestimated detail about the drug.
The window then stays open roughly four to five hours, counted from swallowing rather than from the first kiss.
Start conservatively with the strength
Fifty milligrams is the usual starting point. If that does nothing and was well tolerated, one hundred is the step up — and the ceiling. Twenty-five is for older men, anyone sensitive, and anyone with liver or kidney trouble.
One tablet in twenty-four hours. Two at once raises side effects and nothing else.
Alcohol works against it
A glass of wine is irrelevant. Several are not: alcohol dulls arousal and lowers blood pressure, so the combination tends to make you sleepy rather than capable.
Grapefruit juice is worth skipping for the day. It slows the liver enzyme that clears sildenafil and pushes concentrations up.
The usual side effects share one cause
Headache, a hot flushed face, blocked nose, heartburn. All of them come from vessels widening in places you were not aiming at, and all of them fade with the effect.
Some men see a faint blue cast or find bright light harsh. Unpleasant, harmless, temporary.
When hours count
An erection that will not subside and becomes painful is an emergency — the tissue can be permanently damaged. Sudden vision loss, hearing loss on one side, or chest pain during sex also need help immediately.
One combination you cannot have
Nitrates. That covers nitroglycerin in any form, isosorbide mononitrate, molsidomine, riociguat, and poppers. Both substances widen vessels by the same route, and together the blood pressure fall can be severe enough to stop the circulation working.
No gap between doses makes it acceptable. Anyone prescribed nitrates for angina needs a different approach to erection problems, and a doctor is the right person to find it.
Not for you if
You have had a heart attack or stroke in recent months, have severe heart failure or unstable angina, run very low or poorly controlled blood pressure, or carry an inherited retinal disease. Not after sudden loss of sight in one eye.
Alpha blockers for prostate symptoms need spacing and a low dose. Ritonavir and some antifungals raise levels; rifampicin lowers them.
A failed first attempt proves nothing
It happens constantly. Bad timing, a heavy dinner, or a head that was somewhere else. Several attempts is the fair test before judging the drug.
If it still does not work, another molecule may suit better — tadalafil lasts longer, vardenafil handles food differently.
