What a bottle changes and what it does not

The tablets are the same. Same manufacturer, same strength, same sildenafil. Packaging is the only difference, and it cuts both ways.

In favour: quicker to open, less bulk, and far easier to split a tablet when you want a smaller dose. Against: foil guards each tablet individually against air and moisture, while a bottle only does that collectively, and only while the cap is properly closed.

Splitting is genuinely useful here

Plenty of men do well on half of a hundred-milligram tablet, and a bottle makes that practical. Use a pill cutter rather than a kitchen knife if the tablet has no score line — the halves come out far more even.

Keep the split halves dry and use them within a few days. Exposed surfaces take up moisture faster than coated ones.

What does sildenafil actually do?

Arousal relaxes smooth muscle in erectile tissue and blood flows in. The enzyme PDE5 normaly ends that quickly by breaking down the messenger involved. Sildenafil blocks the enzyme, so the erection lasts.

No arousal, no effect. The tablet clears an obstacle rather than creating desire.

Timing and dose

Take it thirty to sixty minutes ahead with water. Empty stomach is faster; a fatty meal can delay it by an hour or more. The window runs four to five hours.

Fifty milligrams is the usual start, one hundred the maximum, twenty-five the cautious option. One dose per day.

What are the side effects?

Headache, flushing, nasal congestion, heartburn, occasionaly a blue tint to vision or sensitivity to light. All temporary and all from the same vessel-widening effect.

Needing urgent help: sudden vision or one-sided hearing loss, chest pain, or a painful erection that will not go down.

What must never be taken alongside

Nitrate medicines — nitroglycerin, isosorbide mononitrate, molsidomine — plus riociguat and poppers. The two act on the same mechanism, and the combined drop in blood pressure can be fatal rather than merely unpleasant.

This is not a precaution that scales with dose. It applies to the smallest tablet as much as the largest, and it has no safe interval.

Who should not use it

Men with recent heart attack or stroke, severe heart failure, unstable angina, very low blood pressure, severe liver or kidney disease, or an inherited retinal condition. Not for women or anyone under eighteen.

Alpha blockers need spacing. Ritonavir, ketoconazole and erythromycin raise sildenafil levels; rifampicin reduces them. Skip grapefruit juice.
