The dose, stated plainly

One hundred milligrams is the licensed ceiling for sildenafil in a day. This tablet contains half again as much. Nothing about the extra fifty milligrams has been shown to improve results.

What the extra reliably delivers is a stronger headache, a hotter flush, a faster heart rate and more dizziness when you stand up.

Split it, or do not buy it

Two thirds of a tablet is still a full standard dose. Half is a sensible place to begin for most men, and a first-time user should certainly not swallow the whole thing.

Use a pill cutter rather than guesswork. Uneven halves mean you do not actually know what you took, which defeats the purpose of splitting.

Why more is not better

PDE5 inhibition reaches a plateau. Once the enzyme is largely blocked, extra drug cannot block more of it — it just circulates and causes trouble elsewhere in the vascular system.

That is the pharmacological reason a licensed maximum exists in the first place. It is not caution for its own sake.

Nitroglycerin spray or tablets, isosorbide mononitrate, molsidomine, riociguat for pulmonary hypertension, and poppers from the recreational shelf. All of them open blood vessels through the same pathway this drug uses. Put them together and blood pressure can drop far enough to collapse the circulation.

There is no spacing trick that makes this safe. If you carry a nitrate spray for chest pain, this class of medicine is not for you, and that is worth saying plainly rather than burying in small print.

What else interacts with it?

Alpha blockers for prostate symptoms need wide spacing and small doses. Ritonavir, ketoconazole and erythromycin raise sildenafil levels on top of an already high dose.

Contraindications

Heart disease, recent heart attack or stroke, low blood pressure, liver or kidney impairment, inherited retinal disease. If you take daily medication of any kind, ask a pharmacist before the first dose.

If nothing is working at standard doses

Escalating the milligrams is the least likely answer. Check the timing, the meal and the circumstances first. Then consider a different molecule — tadalafil lasts far longer, vardenafil tolerates food better.

And if the problem is new and persistent, have blood pressure and blood sugar looked at. These vessels report vascular disease early.
