What the tablet contains

Tadalafil and dapoxetine together. The first handles erections, the second handles timing, and they work by completly unrelated mechanisms — one on blood vessels, one on nerve signalling.

This is the standard-strength version; Super variants carry a higher tadalafil dose.

The duration gap between the halves

Dapoxetine peaks within an hour and is substantialy cleared within a few. Tadalafil is still active into the next day.

So the delaying effect finishes well before the erectile effect does. The practical consequence: do not take a second tablet the following evening to get the delay back. One in twenty-four hours is the limit, and there is still tadalafil in your system.

The tadalafil half

It inhibits PDE5, keeping the smooth muscle in erectile tissue relaxed while arousal is present, so blood stays where it is wanted. Food has almost no influence on it.

There is a secondary relaxing effect on the prostate and bladder neck, which men with benign enlargement sometimes notice.

What dapoxetine brings, and what it costs you

Dapoxetine is a serotonin reuptake inhibitor built for occasional use — it peaks inside an hour and is largely gone within a few. Serotonin acts as the brake on the ejaculatory reflex, and holding it in place for a while pushes the reflex back.

The price of admission is a specific side effect: a drop in blood pressure on standing, sometimes severe enough to faint. Take it with a large glass of water, stand up slowly, and do not drive. Alcohol makes this considerably worse, so leave it out that evening.

It also rules out a long list of other drugs. Monoamine oxidase inhibitors, thioridazine, linezolid, lithium, St John's wort and other SSRI or SNRI antidepressants do not mix with it, and a gap of weeks is needed either side of an MAO inhibitor. Tramadol and migraine triptans are risky too.

Realistic numbers for the delay

Trials show a lengthening to a multiple of the starting time. In practice, seconds become minutes. Nobody should expect half an hour out of this.

For a man who has barely been getting started, minutes is still a real change — and the drop in pressure often counts for as much.

When should you take it?

One tablet, one to three hours before sex, with a large glass of water. The lead time comes from the dapoxetine component.

Get up slowly afterwards, do not drive, and leave alcohol out that evening.

Side effects from each ingredient

Tadalafil: headache, flushing, blocked nose, reflux, and dull ache in the lower back or thighs that often arrives a day late.

Dapoxetine: nausea, dizziness, dry mouth, insomnia, sometimes diarrhoea. Fainting on standing is the one that causes genuine injuries.

One combination you cannot have

Nitrates. That covers nitroglycerin in any form, isosorbide mononitrate, molsidomine, riociguat, and poppers. Both substances widen vessels by the same route, and together the blood pressure fall can be severe enough to stop the circulation working.

No gap between doses makes it acceptable. Anyone prescribed nitrates for angina needs a different approach to erection problems, and a doctor is the right person to find it.

Who should not take it?

Heart disease, heart failure, conduction disorders, recent heart attack or stroke, unstable angina. A history of fainting, epilepsy, mania or severe depression. Severe liver impairment.

Incompatible with MAO inhibitors, thioridazine, linezolid, lithium, St John's wort and other SSRI or SNRI antidepressants. Ritonavir, ketoconazole and clarithromycin raise tadalafil levels.

The part a tablet does not reach

Early ejaculation usually carries a psychological component, and medication does not touch it. What medication does is remove urgency long enough for other things to be learned.

Breathing, pauses, the stop-start method, a frank conversation with a partner. Unromantic, and they keep working after the drug has gone.
