When vardenafil is the better choice
Three situations come up repeatedly. Sildenafil gave headaches that were not worth the result. Sildenafil gave very little effect at all. Or the lead time of an hour never fitted the way things actually happen.
Vardenafil is chemically related but not identical, so a poor result with one genuinely does not predict the other.

Onset and duration
Usually twenty-five minutes to an hour, often at the quicker end. The window runs around four to five hours afterwards.
A very fatty meal delays it; an ordinary one barely does. That is a practical advantage over sildenafil in everyday use.
The mechanism
Vardenafil inhibits PDE5, the enzyme that would clear away the messenger keeping the vessels in erectile tissue relaxed. Block it and arousal yields a firmer erection that holds.
No arousal, no effect. The same condition applies across the whole class.
Dosing is lower than with sildenafil
Ten milligrams roughly an hour ahead is standard, twenty if ten falls short and is tolerated, five for men who react strongly. Past sixty-five, or with liver problems, start at the bottom.
One tablet in twenty-four hours.
The heart rhythm caution
Vardenafil slighly prolongs the QT interval. For most men that is of no practical consequence, but it matters with antiarrhythmic drugs — amiodarone, sotalol, quinidine — or with a known QT prolongation.
In those cases choose sildenafil or tadalafil instead. This is a real reason to pick a different drug, which is why it is not buried at the bottom of the page.
What can you expect to feel?
Headache first, then flushing, a blocked nose, sometimes dizziness or an unsettled stomach. Vision can flicker briefly or feel over-sensitive to light.
Sudden loss of vision or hearing on one side means stopping and getting seen. A painful erection that will not go down is an emergency.
Nitrates: the hard no
Nitroglycerin spray or tablets, isosorbide mononitrate, molsidomine, riociguat for pulmonary hypertension, and poppers from the recreational shelf. All of them open blood vessels through the same pathway this drug uses. Put them together and blood pressure can drop far enough to collapse the circulation.
There is no spacing trick that makes this safe. If you carry a nitrate spray for chest pain, this class of medicine is not for you, and that is worth saying plainly rather than burying in small print.
Which other drugs matter here?
Alpha blockers for prostate symptoms need spacing and a small dose. Ritonavir and several antifungals raise vardenafil levels sharply. Leave grapefruit juice out.
Who should avoid it
Men with recent heart attack or stroke, severe heart failure, very low blood pressure, severe liver impairment, or on dialysis. Inherited retinal disease rules it out.
Storage
Dry, room temperature, in the blister. Not the bathroom, not the glovebox, and past the printed date they go in the bin.
