Why a third option exists at all

Once sildenafil was widely used, the gaps showed up quickly. Some men got headaches they could not tolerate. Others got little effect. Vardenafil was developed as an alternative — chemically related, not identical.

In practice that means a disappointing experience with one drug does not predict the next. It is a real second chance rather than a marketing line.

Timing

Onset usually falls between twenty-five minutes and an hour, and a fair number of men find it quicker than sildenafil. The window afterwards runs about four to five hours.

A very fatty meal delays the start. An ordinary one barely registers. Pasta is fine; a plate of pork knuckle and fried potatoes is pushing it.

How it works

Vardenafil inhibits PDE5. That enzyme would otherwise break down the messenger keeping blood vessels in erectile tissue relaxed, which is what ends an erection.

Block the enzyme and arousal produces a firmer, longer-lasting erection. Remove the arousal and nothing at all happens.

Doses run lower than sildenafil

Ten milligrams about an hour ahead is the standard. Twenty is available if ten is not enough and is well tolerated; five is the cautious start for men who react strongly.

Over sixty-five, or with liver problems, begin at the lower end. One tablet in twenty-four hours is the limit.

The heart rhythm caution

Vardenafil can lengthen the QT interval slighly — a measure of electrical conduction in the heart. For most men that is of no consequence.

It matters if you take antiarrhythmic drugs such as amiodarone, sotalol or quinidine, or if you have a known QT prolongation. In those cases a different active ingredient is the sensible choice. This is a genuine selection criterion rather than fine print.

What side effects should you expect?

Headache first, then facial flushing, a blocked nose, sometimes dizziness or an unsettled stomach. Vision may flicker briefly or become sensitive to bright light.

Sudden loss of vision or hearing on one side means stop and get seen. A painful erection that does not subside is an emergency, not something to sleep on.

One combination you cannot have

Nitrates. That covers nitroglycerin in any form, isosorbide mononitrate, molsidomine, riociguat, and poppers. Both substances widen vessels by the same route, and together the blood pressure fall can be severe enough to stop the circulation working.

No gap between doses makes it acceptable. Anyone prescribed nitrates for angina needs a different approach to erection problems, and a doctor is the right person to find it.

Which other drugs matter here?

Alpha blockers for prostate symptoms are allowed with spacing and a low dose. Ritonavir and some antifungals raise vardenafil levels sharply, so the dose must come down. Skip grapefruit juice on the day.

Who should avoid it

Men with a recent heart attack or stroke, severe heart failure, very low blood pressure, severe liver impairment or on dialysis. Inherited retinal disease rules it out as well.

If a doctor has advised against sexual exertion for cardiac reasons, that advice stands with or without a tablet.
